In-home physical therapy is also convenient, helps reduce exposure to Covid-19, and is covered under Medicare and most other insurances. We also offer health and wellness programs!

Our programs are designed for the whole person, incorporating proper nutrition and hydration, therapeutic exercise, and home safety into your overall health plan.
We monitor vital signs at every visit, assess any change in medications or condition, and communicate directly with your primary care provider as needed. We can make equipment recommendations including walkers, canes, wheelchairs, cushions, shower chairs, commodes, medication dispensers, lifeline services, stairlifts, and even proper shoes and orthotics!
We work closely with other health care professionals and can recommend other disciplines as needed. We help you navigate the many healthcare services available to you.

Home Safety & Fall Prevention
We take fall prevention very seriously. All of our patients are evaluated for their “risk of falling” regardless of their reason for coming to our practice for Physical Therapy.
Falling is the second leading cause of accidental or unintentional injury-related deaths worldwide. Each year an estimated 646,000 individuals die from falls, and adults over 65 suffer the greatest number of falls.
We will begin with evidence-based screening that includes a standardized Fall Risk questionnaire, balance and gait speed tests, reviewing your history of falls, and assessing the home environment for risks. Following the screening, we will provide recommendations for modifying your home environment to increase safety as well as adding any necessary goals to your therapy plan to reduce your risk of falling.
Health & Wellness Programs
Regular exercise has significant health benefits on your overall health including lowering the risk of diabetes and hypertension, decreasing cholesterol levels, relieving stress, and, more importantly, can improve your overall quality of life. But motivating ourselves to actually do these things can be hard! Our wellness program is perfect for those who have completed their physical therapy course of treatment and wish to continue their health journey with us, or for anyone who wants to improve their overall health beyond specific treatment for illness or injury. Our program includes 1 on 1 coaching with one of our licensed physical therapists who will be your partner in:

Your First Session
You will meet with your Physical Therapist in person at your home. During this session, we will:

We will then conduct a comprehensive evaluation to assess your strength, balance, endurance, posture, pain levels, medications, vitals signs, and home safety.
